In March 2020, a mailing was sent to your home asking for updated medical / dependent verification. The updated paperwork was required to be returned to Richard Gabriel Associates no later than April 30th 2020
Richard Gabriel Associates have not received everyone’s updated paperwork as of today. I have attached a copy of the paperwork. Please fill out and return to Richard Gabriel as soon as possible if you haven’t already done so.
If the information is not received, benefits could be affected.
The form must be completed by ALL members, whether they have insurance through the USW Fund or not, so that the Fund's database records are up to date for census and administrative purposes.
